During the Covid-19 Pandemic, we are simplifying the client referral process by temporarily suspending the requirement to complete a Client Referral Form. Please email a ‘Client Referral’ to, indicating information as follows:
  • Client Name: Last name, first name
  • Client Telephone Number
  • Indicate on subject line the location where the client will pick up the Quest Client Card (611 Hastings, Dundas, Burnaby, Surrey, North Vancouver)
  • It is very important that your email signature has your contact information, address, telephone, email and name of organization
  • Your email should be your organization’s email address
48 hours after receiving acknowledgement of your email, the client can go into their specified market to pick up their client card. Please send referrals to
